 Who Is it For?

Chronic disease-specific education is for people living with the following chronic conditions:

  • Diabetes
  • Stroke
  • Heart Disease
  • COPD
  • Asthma


  • Education is specific to the individual and his/her family.
  • Educators can refer to many other community programs and professionals.

Do You Need a Referral?

No. You can refer yourself to the program or you can be referred by your physician or other health care professional.

How to Register:

Please phone the location most convenient to you or ask your health care provider for a referral.

Disease-specific education is offered Free of Charge.
